Membership in Duke Connected Care gives independent practices access to Duke’s payer strategy, quality and value-based-care support, and referral connectivity — plus a defined set of enhanced services such as Epic (Maestro Care), remote patient monitoring, and care management — without changing how the practice is owned or run.

- This is a defined, light-touch set: many operational and digital-ops services are not included at this level (see the exclusions below and the comparison matrix).
- Duke’s fully negotiated MSO payer rates are not extended at the practice level here — Connect provides payer strategy and CIN-based contracting access; practice-level negotiated rates scale with deeper integration (see Integrate).
